Your Pregnancy Journey at Paramitha โ Week by Week
First Trimester (Weeks 1โ12): Laying the Foundation
We recommend registering at Paramitha as soon as your pregnancy is confirmed โ ideally before 8โ10 weeks. Early registration allows our team to establish your baseline health profile, assess risk factors, order first-trimester blood tests, and book your NT scan at the right gestational window (11โ13 weeks + 6 days).
Your first antenatal blood panel includes CBC, blood group and Rh typing, blood glucose, TSH, VDRL, HIV, Hepatitis B surface antigen, and urine culture. The NT scan, performed between 11 weeks and 13 weeks 6 days, provides an individualised risk assessment for chromosomal conditions when combined with PAPP-A and beta-hCG blood tests.
Morning sickness affects the majority of pregnant women. Paramitha's obstetricians provide dietary guidance, safe anti-nausea medication when required, and intravenous hydration for severe cases (hyperemesis gravidarum).
Second Trimester (Weeks 13โ27): Growth and Surveillance
The mid-trimester TIFFA scan (18โ22 weeks) systematically evaluates the baby's brain, heart, spine, abdominal organs, kidneys, limbs, and placental position. Between 24 and 28 weeks, all pregnant women are offered the Oral Glucose Tolerance Test (OGTT) for gestational diabetes.
Anaemia screening and iron supplementation are provided at every trimester. From 28 weeks onward, mothers are taught structured kick count methods โ any reduction in fetal movements is investigated promptly with CTG and ultrasound.
Third Trimester (Weeks 28โ40+): Preparing for Birth
Growth scans and Doppler studies track fetal weight, amniotic fluid, and placental function. GBS screening is offered at 35โ37 weeks. At 36โ37 weeks, your obstetrician holds a structured birth planning consultation covering presentation, pain relief preferences, and contingency planning.

Delivery Services in Detail
Normal Vaginal Delivery
Paramitha strongly supports and promotes normal vaginal delivery for eligible mothers. Labour rooms are equipped with continuous CTG monitoring, experienced labour room nurses, birthing balls, adjustable beds, and individual privacy during active labour.
Our obstetricians practise active management of the third stage of labour (AMTSL). Episiotomy is performed only when clinically necessary. Delayed cord clamping and immediate skin-to-skin contact are standard practice.
Painless Delivery (Epidural Labour Analgesia)
- The mother remains fully awake and alert throughout labour and delivery.
- She retains the ability to push during the second stage.
- Fetal monitoring continues throughout.
- Epidural analgesia does not increase the risk of C-section.

Caesarean Section (C-Section)
- Planned (elective) C-sections scheduled at 39 weeks or later unless medically indicated earlier.
- Emergency C-sections performed within 30 minutes of decision-to-incision for Category 1 emergencies.
- Regional anaesthesia (spinal or epidural) used for most C-sections.
- Partner permitted in OT during planned C-sections (confirm branch policy at registration).
- Skin-to-skin contact in the OT facilitated wherever clinically stable.
Vaginal Birth After Caesarean (VBAC)
Mothers who have previously delivered by caesarean are evaluated for VBAC eligibility based on the reason for the previous C-section, uterine incision type, inter-delivery interval, fetal size, and maternal factors. Eligible women are supported with continuous intrapartum monitoring.
Breech Pregnancy Management
When a baby is in breech position at 36+ weeks, options include External Cephalic Version (ECV), planned vaginal breech delivery in selected cases, or planned caesarean section โ always individualised with informed consent.
Postnatal Care
- Postnatal ward monitoring of vital signs, uterine involution, wound healing, and emotional wellbeing.
- Breastfeeding initiation in the delivery room with skin-to-skin contact within the first hour.

Hypertension in Pregnancy & Pre-eclampsia

Twin and Multiple Pregnancies
Managed with more frequent scans and a clearly defined delivery plan. Level III NICU prepared to receive premature twins.

Structured investigations including antiphospholipid antibody testing, uterine anatomy assessment, parental chromosomal analysis, and thrombophilia screening. Additional surveillance and targeted therapy where indicated.
Thyroid Disorders in Pregnancy
Hypothyroidism and hyperthyroidism require careful monitoring and dose adjustment. Thyroid function checked at every trimester in women with known thyroid disease.
Placenta Praevia and Accreta Spectrum
Serial ultrasound monitoring, activity restriction, anaesthetic counselling, and planned caesarean section. Placenta accreta cases coordinated with experienced surgical specialists and blood bank support.
